• At least 55 years of age.
• A resident of Onondaga County
• Assessed by our interdisciplinary team and certified by the Onondaga County Social Services Resource Center to be eligible for a nursing facility level of care.
  

Additionally you must meet the following conditions of enrollment:

1. Determined to be capable of safely residing in the community with PACE CNY support services at time of enrollment.
   
   
2. Qualify for Medicaid and/or Medicare or agree to pay private pay amounts set forth in the Enrollment Agreement.
   
   
3. In need of the long term care services of the plan for more than 120 days.

Enrollment/Disenrollment:

To enroll in PACE CNY one must meet eligibility criteria on enrollment and annual recertification. A PACE CNY participant may voluntarily disenroll from the program without cause at any time.

A PACE participant may not enroll or disenroll at the Social Security Administration office or by mail.

A PACE participant must receive all needed health care, including primary care and specialist physician services (other than emergency services) from PACE CNY or an entity authorized by PACE CNY.

Participants may be fully and personally liable for the costs of services not authorized through the PACE CNY program (other than emergency services).
